Cholecalciferol (Vitamin D3) ist die inaktive Vorstufe des aktiven Calcitriol und wird in der Haut durch UVB-Strahlung aus 7-Dehydrocholesterin gebildet. Topisches Vitamin D3 wird von Keratinozyten über die 25-Hydroxylase und 1α-Hydroxylase lokal in aktives Calcitriol umgewandelt. Es reguliert die Keratinozyten-Proliferation und -Differenzierung, moduliert das angeborene und adaptive Immunsystem und besitzt antiinflammatorische Eigenschaften. Klinische Studien belegen die Wirksamkeit bei leichter bis mittelschwerer Psoriasis, wo es die Schuppenbildung reduziert und den Juckreiz lindert. Vitamin D3 unterstützt zudem die Produktion von Cathelicidin und Defensinen, natürlichen antimikrobiellen Peptiden der Hautbarriere. Ein Vitamin-D-Mangel korreliert mit erhöhter Inzidenz von Atopischer Dermatitis und Ekzemen.
