Dark spots are the number one complexion complaint dermatologists hear — and the skincare industry sells hundreds of "brightening" products that do nothing for them. Fading unwanted pigment is actually well understood dermatology. Here is what genuinely works, in order of evidence, with US product picks.
| Type | Cause | Treatment Response | |------|-------|--------------------| | Post-inflammatory hyperpigmentation (PIH) | Healed acne/eczema/injury | ✅ Very good with topicals | | Sun spots (solar lentigines) | Chronic UV exposure | ✅ Good with topicals + prevention | | Melasma | Hormones + UV (+genetics) | ⚠️ Stubborn; needs combination therapy | | Freckles | Genetics + UV | ⚠️ Fade with sun avoidance, return with exposure | | Post-inflammatory erythema (PIE) | Dilated capillaries (red, not brown) | ❌ Topicals barely help; vascular laser fixes |
The red vs. brown distinction matters enormously: PIE (red marks, common in lighter skin after acne) is a blood vessel problem — no cream fades it well. Brown pigment is a melanin problem — creams can fade that.
Lemon juice (irritant, phototoxic), scrubbing (worsens PIH), most "brightening" masks with no active at effective concentration, and — critically — any routine without sunscreen.
Every melanocyte-stimulating signal you block matters more than any fade ingredient you apply. One unprotected beach day can undo months of fading. Dermatologists' rule: no sunscreen, no results.

Expect first visible fading at 8–10 weeks, substantial improvement at 4–6 months. Anyone promising faster is selling hydroquinone unsupervised or lying.

PIH in Fitzpatrick IV–VI skin is more common, darker, and slower to fade — and aggressive treatment can itself cause new PIH. Dermatologists' guidance for deeper tones:
A sun spot treated consistently (active + sunscreen) fades noticeably in 2–3 months and may clear in 6 months. PIH from acne takes 3–6 months on average. Melasma is managed, not cured — maintenance is lifelong.
The ones with proven actives at effective strengths do: azelaic acid 10%, tranexamic acid 3%+, retinoids, niacinamide 5%, L-ascorbic acid 10%+. Products relying on botanical extracts alone rarely move the needle.
They can remove them dramatically: chemical peels, Q-switched/laser treatment, cryotherapy for lentigines. "Permanence" depends on sun behavior afterward — without SPF, new spots form. Melasma usually recurs with UV and hormones even after laser.
Hydroquinone is not banned, but since 2020 the CARES Act restricted OTC sales — it is now prescription-only. It remains an effective prescription option under dermatologist supervision, typically in short courses.
Almost always sun exposure. Melanocytes have memory: UV re-triggers the same spots. Daily SPF 30+ is the only reliable prevention.
